We provide care and services for senior citizens living independently and to those who need assistance with daily living activities or full nursing care.
Our social worker is key to providing support to senior adults as they age and to their families.
Responsibilities:
Provide support services to residents Provide support services to family members and significant others of residents Complete pre-admission assessments of prospective residents.
Complete assessments as needed for each resident - i.
e.
:
social history, MSE, MMSE, UAI, MDS, etc.
Participate in care plan process for residents Provide information regarding Advance Directives, DNR, DDNR and choices for care direction Participate in discharge and transfer planning for residents Understand all levels of care and services offered to include rates, covered services, insurance coverage (to include Medicare and Medicaid) and be able to communicate to residents and their families

Skills and Experience for Success:
Social Work Degree (or degree in related field) Previous experience in long-term care a plus Work with a multi-level caseload that is not always predictable Work independently Quick Apply.
Estimated Salary: $20 to $28 per hour based on qualifications.

The check will bounce and the victim is left responsible.
